The crash occurred Wednesday morning on a curve on Atlanta Road near Seaford.
Police say 36-year-old Roger Warner of Seaford, the driver of one car involved, was pronounced dead on the scene and 7-year-old Dewayne Eastburn of Federalsburg, Md., died at Johns Hopkins Trauma Center in Baltimore.
The driver of the other vehicle was taken to Nanticoke Hospital and underwent emergency surgery for internal injuries.
Police say four children, aged 6 to 11, are in serious condition and three of them were not wearing seat belts.
