The Robert Wood Johnson Foundation looked at factors like smoking, binge drinking, access to doctors and childhood poverty to rank counties in each state.
In Pennsylvania, Philadelphia was at the bottom, 67th, while Chester County was number 1, or the healthiest.
Montgomery was 5th, Bucks was 7th, and Delaware County was 36th.
In New Jersey, Cumberland County was at the bottom, Hunterdon was at the top.
In Delaware, New Castle County came out on top.
The authors of the report hope county leaders will use the ranking to identify and address their specific health problems.
