Christina Gamble, 43, faces theft and insurance fraud charges.
The office of the Pennsylvania Attorney said that in November, 2007, Gamble reported that she slipped while working as a waitress and allegedly injured her back.
The claim noted that Gamble told her doctor the injury left her unable to work.
Initially her claim was denied but in October 2008 a judge awarded her workers compensation, and she was given back pay.
The following month an investigator from Highmark Insurance got a tip from a former co-worker that Gamble was working as an exotic dancer in Easton.
The investigator went to C.R. Fanny's Gentleman's Club and Sports bar and, the A.G. said, obtained surveillance video of Gamble dancing.
The insurance company stopped Gamble's benefits in January, 2009. She was questioned by investigators in March, 2010.
Investigators say Highmark paid nearly $23,000 in benefits and more than $4,000 in medical expenses while Gamble was on workers compensation.
Gamble faces a maximum penalty of seven years in prison and a $15,000 fine for each charge. She faces a preliminary hearing in May.
