PHILADELPHIA (WPVI) -- Philadelphia police have released new details about last week's fatal police shooting outside St. Christopher's Hospital for Children.

The officer who fired his weapon has been identified as 28-year-old Officer Azieme Lindsey.

According to police, officers responded to the hospital around 10:21 a.m. on July 2 for a report of a person with a weapon. Authorities said a family member of Joseph Smithers, 32, called 911 because they were concerned Smithers was experiencing a mental health crisis and was suicidal.

Police said Smithers had been turned away from the hospital around the same time the 911 call was made. Investigators also said he had been asked to leave the property the previous day.

Officer Lindsey and a second officer responded and located Smithers at a nearby bus stop with a 29-year-old woman.

As Officer Lindsey exited his patrol vehicle and approached, police said Smithers began backing away while removing a handgun from his waistband.

Authorities said Lindsey then fired his department-issued weapon, striking Smithers multiple times.

Smithers was transported to Temple University Hospital, where he was pronounced dead.

The woman suffered a graze wound to the side of her neck during the incident. She was taken to Temple University Hospital and treated for her injury.

Police said investigators recovered a handgun next to Smithers. Authorities said the firearm was legally registered to him and that he possessed a valid license to carry.

The second officer on the scene did not discharge a weapon.

Officer Lindsey has been placed on administrative duty while the investigation continues.

Police said body-worn cameras captured the incident.
