Police say 47 year old Anthony Consalvo was driving his minivan when he crossed over into oncoming traffic and slammed into a truck.
The accident happened around midnight at the Henry Avenue overpass in East Falls.
His 36-year-old passenger was rushed to the hospital with critical injuries.
Consalvo and two people in the truck are in stable condition with cuts and bruises.
The road was closed for several hours but has reopened.
The investigation into the accident continues.
