June 16, 2014 (WPVI) -- Penn Medicine's Comprehensive Longitudinal Advanced Illness Management (CLAIM) Program is an evidence-based approach to comprehensive home care developed through extensive preference assessment research, pilot-testing, and evaluation.

The overarching aim of CLAIM is to provide in home support to patients with advanced cancer. Through symptom and pain management, care coordination, and triage, CLAIM gives patients the opportunity to remain in their homes and avoid unnecessary and undesirable hospitalizations.

As part of a comprehensive take on long-term cancer care, CLAIM can help provide the benefits of a 24-hour on call nurse, spiritual and emotional support for patients and their families with a social worker or chaplain, as well as advice about medication dosage and side effects.

The CLAIM in-home clinical team can keep emergency room visits and unplanned admissions to the hospital to a minimum by meeting patients' individualized care needs, like wound care, pain and symptom management, and medication education, in addition to performing routine check-ups and tests.

Meghan O'Regan Coleman DNP, RN, CRNP, AOCNP - Meghan Coleman is a Nurse Practitioner at Penn Wissahickon Hospice and Home Care. She completed the Master of Nursing/Adult Oncology Nurse Practitioner program at the University of Pennsylvania and received a Doctorate of Nursing Practice at Villanova University. She has been working with patients in the Philadelphia and New Jersey area for over ten years.

Jamee Roberts LSW - Jamee Roberts is a Licensed Social Worker for Penn Home Care and Hospice Services. She received her Master's in Social Work from the University of Pennsylvania in 2010, and has been actively involved in committees within the Penn Home Care and Hospice Services organization to encourage continued education for fellow team members.

David Casarett MD MA - Dr. Casarett is the head of the CLAIM program and the Director of Hospice and Palliative Care for Penn Medicine. Dr. Casarett is involved in numerous research endeavors aimed at giving cancer patients a better quality of life.

Susan Foster RN BSN CHPN - Susan Foster is the Associate Director of Penn Wissahickon Hospice, a division of Penn Home Care and Hospice Services. She has been working in hospice care for over 20 years.
