The accident happened on the southbound side in Kenilworth Monday night, right before exit 138.
A tire flew off a car and plunged through the windshield of a white Hyundai Sonata, killing the driver.
Russell Seratelli, 47, was pronounced dead at the scene. His 25-year-old daughter was in the passenger seat and was not hurt.
The accident snarled traffic for five miles during the evening rush hour.
