WYNDMOOR, Pa. (WPVI) -- State authorities have shut down a day care in Wyndmoor, Montgomery County has been shut down following the death of a child.
Investigators from the Department of Public Welfare conducted an inspection at the Wyndmoor Montessori School (named in documents as the Wyndmoor Learning Center) at 1400 E. Willow Grove Avenue on Monday.
During that inspection, investigators say they reviewed surveillance video that allegedly showed a staffer putting a sheet over the child's face.
It's not clear what happened next, but paramedics were called and the staffer was allegedly seen trying to hide the sheet.
Furthermore, investigators said, notification to parents that the child was being taken to the hospital was delayed because workers had to search for contact information.
The date of the incident and the exact manner of the child's death were not immediately available.
The DPW said during the investigation they found numerous other violations relating to child safety and staff training.
In the document ordering the facility to stop caring for children said it demonstrated "gross incompetence, negligence and misconduct in operating a facility likely to constitute immediate and serious danger to the life or health of the children in care."
That order was issued earlier this week.
