It happened on Route 13 and Edgely Road shortly before 10:30 Friday night.
Police say a 2006 KIA driven by Monica Lynn Presley, of Falls Township, was struck from behind by a 1995 Ford van operated by Johnathan Fitzgibbon, also of Falls Township.
The van left the road and struck a PECO pole, a traffic light, and a parked car.
Presley died at the scene. A passenger in Presley's car and Fitzgibbon were taken to Saint Mary Medical Center to be treated for minor injuries
.Police continue to investigate the accident.
